BariatricPal Ultra Multi Bariatric Multivitamin & Bariatric Eating J2 Multivitamin Iron Bottles

BariatricPal & Bariatric Eating, in partnership with the U.S. Consumer Product Safety Commission (CPSC), is voluntarily recalling certain bottles of an iron-containing BariatricPal & Bariatric Eating dietary supplements manufactured by VitaQuest International because they do not have child-resistant packaging as required by the Poison Prevention Packaging Act (PPPA). The bottle caps are not child-resistant, posing a risk of serious or fatal poisoning if the contents are swallowed by young children.
This recall involves bottles of one BariatricPal dietary supplement and one Bariatric Eating dietary supplement: BariatricPal Ultra Multi Bariatric Multivitamin (90-count bottles), which contain 45 mg of iron per serving (15 mg per capsule) and Bariatric Eating J2 Two Per Day Berry Bariatric Chewable Tablets (60-count bottles), which contain 18mg of iron per serving (9 mg per capsule).
Importantly, the recall is limited to the packaging of the products (specifically the bottle caps) and not the multivitamins themselves.
The recalled bottle caps can be identified because they are smooth on top, and do not have any embossed lettering instructing users to “push down and turn”.
The recalled bottles include lot numbers 0148I4 and 0279K3. In addition to visually inspecting the cap to determine whether your unit is affected, the lot number is printed in black ink on the bottom of the bottle.
- Only a subset of units have non-conforming caps. If a bottle marked with the above lot numbers have a child-resistant cap, it is not part of the recall.
What You Should Do: Please immediately secure the recalled bottles out of sight and reach of children, and fill out the form below to obtain a free replacement child-resistant cap. When you receive the child-resistant replacement cap, please substitute it for the recalled cap and dispose of the recalled cap.
